Clinical Test Request Forms Updated

January 9, 2024 -- "Amerigroup Iowa MCO" changed their name to "Wellpoint Iowa MCO" effective Jan. 1, 2024. SHL has updated their clinical test request forms to reflect this. Several changes have also been made to the Bacteriology Test Request Form. Please download the new versions of these test request forms and destroy any copies of old versions.
