Laboratories Certified for Any Specified Groups

This form generates a report listing certified laboratories that match the groups that are selected. For a laboratory to be qualified, it must match ANY of the selected categories. For example, selecting SDWA-IOC and SDWA-VOC would produce a listing of laboratories that are certified for either SDWA IOC or SDWA VOC.

Furthermore, this report will list only the selected groups for each laboratory. Hence a laboratory may be certified for additional programs/groups outside the selected criteria which will not be included on the report.

  1. Select the fields for your report.

  2. Select the groups and analytical-series for your report.

  3. Limit your selection by specifying Field-Office or State. (optional)

In order to print this report, you will need to change the setting on your printer from portrait to landscape orientation.
