SHL updates Clinical Test Request Forms

Test request form buttonThe State Hygienic Laboratory (SHL) is implementing new Clinical Test Request Forms (TRFs) with updates to the insurance section, effective immediately.

To: Iowa clinical laboratories

Purpose: The State Hygienic Laboratory (SHL) is implementing new Clinical Test Request Forms (TRFs) with updates to the insurance section, effective immediately.

Details: Iowa’s Medicaid program is now managed through three statewide managed care organizations (MCOs); a change that went into effect on April 1, 2016. SHL’s updated TRFs reflect this change and improve the ability to communicate pertinent patient insurance information with the laboratory order.

Guidance: To access the form, visit SHL’s homepage at www.shl.uiowa.edu and click on the Clinical Test Request Forms button. New TRFs have the code “042016b” in the lower left corner.

To bill the test to the patient’s MCO/Medicaid/Medicare account:

  1. Check the appropriate public insurance box
  2. Enter the patient’s Medicaid or Medicare ID#
  3. Enter the Diagnosis Code
  4. Complete the Ordering Health Care Provider Information
