Clinical forms add insurance section
The Hygienic Laboratory’s clinical test request forms have been updated to include insurance information related to Iowa’s transition to a statewide managed care system for Medicaid. To access the new forms, select the “Clinical Test Request Form” icon on the home page, which links to a page with a menu of tests and health care facilities in Iowa. Choose the name of the test and of the facility, and then select the submit button at the bottom of the page. A form with the updated information will be generated electronically for the select health care facility.
